Orthodontics for Children, Teens, and Adults in Manhattan

Karpov Orthodontics provides personalized treatment for patients at every stage of life. Care may focus on alignment, jaw development, skeletal balance, comfort, or sleep-related concerns. Below is an overview of the areas we specialize in, with links to learn more about each option.

Early Treatment (Children)

Early evaluation allows us to identify developing concerns before they become more complex. Treatment may be recommended before age 7 if a child shows signs such as difficulty chewing, mouth breathing, crowding, abnormal bite development, jaw clicking, speech concerns, or habits like thumb sucking.

Early guidance can support healthier jaw growth and reduce the need for more extensive care later.

Adolescent Treatment

Most full treatment for adolescents begins around ages 12–13, once permanent teeth have erupted. Treatment time typically ranges from 18–36 months, depending on individual needs and compliance.

We use modern self-ligating braces, clear brackets, and digitally planned systems such as Insignia to support efficient, comfortable tooth movement.

Adult Treatment

Many adults seek treatment for crowding, bite imbalance, jaw discomfort, or wear on the teeth. Alignment can improve comfort, function, and long-term dental health at any age.

Options may include metal or ceramic braces, lingual braces, or clear aligners such as Invisalign, planned with attention to bite balance and stability.

TMJ & Jaw Discomfort

Jaw tension, headaches, facial pain, or clicking may be related to bite imbalance or jaw joint strain. Evaluation may include imaging to better understand joint position and function.

Care often involves splint therapy to reduce strain and stabilize the chewing system.

Expansion Therapy & Airway-Related Concerns

A narrow palate or low tongue posture can contribute to mouth breathing, poor sleep quality, crowding, and jaw imbalance. Expansion therapy widens the upper jaw and nasal floor, often supporting improved airflow and balanced function.

Care may involve palatal expansion, myofunctional therapy, or coordination with other specialists when appropriate.

Palatal Expansion by Age

Child Expansion (RPE)

Rapid Palatal Expansion is used in younger children while growth plates are still flexible. Expansion typically occurs over several weeks, followed by a retention phase to stabilize new bone.

Teen Expansion

In teens, bone-anchored expanders allow expansion with reduced stress on the teeth. This is often coordinated with braces or aligners as part of comprehensive care.

MARPE & Maxillary Skeletal Expansion (MSE)

MARPE and MSE allow non-surgical widening of the upper jaw in teens and adults using mini-implant–supported expanders. Digital scanning and 3D imaging are used to design precise, customized appliances.

Expansion typically occurs over several weeks and may be followed by braces or aligners.

Surgical & Adjunctive Therapies

Orthognathic Surgery

In some severe skeletal cases, jaw surgery combined with alignment may be necessary to achieve proper balance and function.

Corticotomy & SFOT

These procedures may be used to shorten treatment time, improve bone support, or address complex cases, sometimes as an alternative to jaw surgery.

Functional Appliances (Herbst)

The Herbst appliance is used in growing patients to guide jaw position in cases where the lower jaw is set back. It can improve bite balance, facial profile, and, in some cases, mouth breathing.

Digital Planning & Technology

We use digital treatment planning systems such as SureSmile, along with custom wires, aligners, and modern expansion techniques, to support precise and efficient care.
